Care Advisor / Case Manager - Home Care Packages

The Opportunity

Join a leading community health service located in Melbourne's north. Work as a Home Care Packages Care Advisor (Case Manager) with frail aged people living in the local community. The position is a fulltime permanent role paying an above award salary with full salary packaging. Work in a supportive team that is committed to ensuring high quality service for clients.

Skills and Experience

To be considered for this position you must have recent experience as a Case Manager / Care Advisor in Community Aged Care - either in Home Care Packages, Regional Assessment Service or another similarly funded program.

Also essential are the following -

  • Relevant qualifications at a Diploma level or above
  • Administrative skills
  • Ability to work both in a team and autonomously
  • National police check or willingness to apply for one
  • Working with children check or willingness to apply for one
  • Drivers licence
  • Willingness to undertake a check on Disability Workers Exclusion Scheme
  • Second language is well regarded
