This role works with participants with an acquired brain injury leaving rehab and puts in place supports through NDIS funding to allow them to live in the community rather than entering into a residential aged care environment. Through a Collaborative Discharge approach, you will work with other services including hospital rehab wards. As a Support Coordinator specialising in this area, you will work with participants helping them to utilise funding on their NDIS plan to live their best life after a significant brain injury or stroke.

The successful candidate
To be considered for this role you must have an understanding of the rehab process, neurological disability and the NDIS support coordination role. We are looking for a candidate with a background in nursing or allied health who has worked in the rehab environment and who has experience in NDIS support coordination or a candidate who has had specific case management experience with people with an ABI.
