As Client Advisor for this highly regarded Not for Profit In Home Care provider, your key role will be to develop and nurture positive interactions between clients, carers and local service providers.This is a 12 month contract role, working from home during COVID-19

To be successful in this role you must have:

  • At least 12 months experience in the delivery of quality,client focussed Aged Care case management
  • Demonstrated understanding of Aged Care funding sources, especially Home Care Packages
  • Experience developing Care Plans for complex clients
  • Current Victorian Drivers Licence
  • Tertiary qualifications in Community Services or related disciplines
