Based in Morwell and servicing the surrounding regional areas, your key role will be to develop and nurture positive interactions between clients, carers and local service providers.

To be successful in this role you must have:

  • At least 2 months experience in the delivery of quality,client focussed case management
  • Demonstrated understanding of Aged Care funding sources
  • Experience developing Care Plans for complex clients
  • Current Victorian Drivers Licence
  • Tertiary qualifications in Community Services or related disciplines
