About the organisation

Based in Richmond, a progressive industry leader, supporting clients who require community care services, frail aged clients and clients with a disability that live in the inner eastern suburbs.


  • Co-ordinating services using HACC Pack such as Home Care, Personal Care, Respite care, community transport, PAG, Home Maintenance services to clients who have been assessed as eligible for CHSP or HACC PYP funding.
  • Supporting carers while they are working out in the community, assisting with phone queries around OHS, client requests etc.
  • Dealing with community care clients and their families over the phone in a friendly, professional manner, confirming services, answering queries.
  • Access MAC and work on intake / assessment of new referrals
  • Referral of issues or changes to clients situation to manager, care plan adjustments.

Skills and experience

As this is a locum position, we require a CHSP Review Officer with at least six months experience working in the community care sector, with either LG or NFP organisations, you must be fluent in a care management software package, such as Carelink Plus, Goldcare, Procura or TCM would also be acceptable. A qualification that is relevant in the community care sector is essential. You will need a good working knowledge of MAC and an understanding of CHSP funding. The role is office based, so strong administrative skills are required. To apply you will need a current police check and WWC check.
