We are seeking a Support Co-ordinator to commence ASAP in a locum role that has potential for permanency. This role will be full time, based out of Ballarat, working with clients in the Ballarat / Central Highlands / Grampians region.

Support co-ordinators support clients with a disability by providing holistic and comprehensive services to increase their opportunities of enhanced quality of life and community inclusion. This includes assessments, implementing and reviewing plans and goal setting, monitoring progress, advocacy and referrals.

You will need:

To apply, you will need to have appropiate tertiary qulaifications in Disability and at least 3 years experience working in Support co-ordination/case management roles and be able to demonstrate a contemporary understanding of disability and knowledge of complex conditions that impact on disability and the ability to remain up to date with related developments and a sound understanding of NDIS.

Highly desirable is a strong background of working with highly complex clients, demonstrated management of clients with secondary mental health issues, alcohol and substance abuse issues, homelessness, attendance at VCAT hearings and dealing with child protection.

You will need to have a current Police check, and be willing to undergo a DWES check and have a current WWC check.
