  • Support clients by providing holistic and comprehensive services to increase their opportunities to live a life of choice and inclusion.
  • Provide information to clients, their families and carers on procedures, practices and guidelines.
  • Support the implementation and review of client support plans, programs and services.
  • Liaise with agencies, such as courts, policy and mental health services and communicate effectively with external agencies
  • Accurate record keeping of your works activities in accordance with legislative requirements

Skills and experience

To apply for this role, you would need proven experience with stakeholder management , influence and negotiation and problem solving in order to achieve best outcome for your clients. A strong Disability Case management background, with experience using a care management software program. You will possess a sound understaning of brokerage models, and a working knowledge of NDIS. Tertiary qualifications in nursing, allied health or social sciences are required.
